I, on behalf of the owner of the above Premises, apply to the City of Sherwood for water/sewer/storm water services(s) to the Premises described above. In consideration of said services being furnished, I agree – on behalf of the Premise’s owner – to the following:

That Owner shall be responsible for all services and fees associated with the City’s service(s) provided to the Premises as of the date of this application and remain so until such time as sufficient notice is given the City for discontinuance of the service(s).

That the Owner, their successor(s) and assigns shall be bound and abide by and comply with any and all rules and regulation now in force or hereinafter adopted by the City relating to delivery of service(s) to the Premises.

That the Owner shall be responsible for all charges for service to the Premises and shall ensure that said charges are paid prior to becoming delinquent.

That if this application for service is accepted by the City, the application constitutes an agreement between the Owner and the City.

Delinquency: The City reserves the right to discontinue service(s) to the Premises when charges are not paid when due and refuse further service(s) until all outstanding charges for City service(s) have been paid in full.

Failure to Pay: Failure to pay all charges for service(s) when due may result, among other remedies available to the City, in the termination of service(s) to the Premises and the initiation of further collection efforts the costs of which the Owner shall be responsible in addition to the failure to pay charges for service(s).
