Test Proctoring Request Form and Guidelines
Sherwood Public Library offers test proctoring by appointment.
Please read through the guidelines on this page and complete the Proctoring Request Form below to request a proctoring session. We look forward to helping you with your educational goals.
Student Requirements
- Appointments must be scheduled at least one week in advance.
- Test takers do not need to be library cardholders or residents of a specific geographic area.
- Make all arrangements with the institution giving the exam. If your school requires a proctor form to be filled out, please email the form to SPLProctoring@sherwoodoregon.gov.
- Contact your institution to verify that our proctoring procedures meet your institution’s testing requirements.
- Arrange for the delivery of the exam. Library staff cannot make these arrangements for you.
Proctoring Fees
- There are no fees for proctored exams, however, for exams that need to be returned via methods other than standard USPS or scanning/emailing, the test taker may be required to provide postage and/or appropriate mailing envelopes (e.g. high priority, certified, UPS, FedEx, etc.).
Proctoring Services
- Exams may be handwritten or online.
- Students must use their own laptops for online exams.
- Library staff will ensure the person taking the exam meets all requirements (e.g. verifying ID and whether they have any accompanying materials).
- Cell phones, calculators, backpacks, and other items you bring with you that are not specifically allowed for testing will be held securely for you during the testing period.
- Library staff are not able to personally oversee the test taker at all times. It is the student’s responsibility to inform the examining institution of this restriction.
- Students are seated on the main floor of the library during the exam. We cannot guarantee that the library will be quiet and free of distractions. We can make recommendations about times that may be quietest.
- Exams received by the library and not taken by the student within one month will be discarded.
- Library staff will determine available resources to proctor an exam and may decline to proctor if no designated staff are available or the exam is not compatible with the library’s procedure.
- Student’s are strongly encouraged to schedule testing at least two weeks before the test due date.
