Thank you for initiating a new utility account with the City of Sherwood. Please complete this form and submit within 5 days of receipt. Completion of this form and agreeing to the terms of the agreement will finalize your new account.

If you HAVE NOT previously spoken with a Customer Service Representative to initiate your Utility Account please do not submit this form.  Please call 503.925.2315.

Note: There is a non-refundable account activation fee which will appear on the first bill.
