Police Department Forms
Fingerprinting services (ink only), are available by appointment only. Once the Fingerprint Request Form is completed, please either fax to 503-925-7159, e-mail to burkes@sherwoodoregon.gov, drop by or mail to the City of Sherwood Police Department. You will be contacted and an appointment set up. Cost is $28.05 per card.
The Sherwood Police Department Administrative Office is open M-F from 8:00 a.m. until 5:00 p.m., excluding holidays. Please allow a minimum of 24 hours to process your report request.
The Sherwood Police Department's Community Room is available for not-for-profit organizations, governmental and intergovernmental agency use M-F, 9-4. Please call 503-625-5523 for availability before completing / submitting forms.
